Senior Health Success Stories: How FOX Rehab and Coterie Hudson Yards Help Residents Achieve New Goals

“Our mission is to abolish ageism,” says Irene Blumenthal, Director of Senior Living Partnerships at FOX. “Just because somebody is 80 or 85 or 90 years old doesn’t mean they can’t still achieve new goals that they set for themselves.”

Founded in 1998, FOX Rehab is a clinician-operated, professional private practice specializing in physical, occupational, and speech therapy for older adults. “We take time to get to know residents so we can focus on setting goals that are important to them,” Blumenthal says. “The more personally motivated they are in their therapy sessions, the more successful the end result tends to be.”

FOX also publishes personalized success stories for residents and their families whenever a resident achieves one of their goals, with photos highlighting how far they’ve progressed on their rehab journey.

“We think it’s important for residents and family members to see how these comprehensive therapy services and exercise programs have tangible, positive results for their quality of life,” Blumenthal says.

FOX’s collaboration with Coterie Hudson Yards is centered around two of FOX’s signature senior health programs: Restoring Balance, which focuses on senior fall prevention, and Restoring Life, which provides compassionate and results-based therapy services for memory care residents living with Alzheimer’s and dementia.


Restoring Balance – Senior Fall Prevention


Coterie takes senior fall prevention seriously – for many older adults, a simple fall can lead to injury, loss of independence, and long-term health complications. FOX’s Restoring Balance program takes a wellness-driven, evidence-based approach to reducing fall risks by improving strength, balance, and overall mobility.

Alongside Coterie’s nursing team, FOX’s physical therapists provide a baseline fall risk assessment for each resident when they first move in; additional assessments are performed whenever a resident returns from an inpatient visit or reports a notable change in their balance or mobility. The goal of these assessments is to identify and address any underlying issues such as muscle weakness or poor posture.

Once a resident’s specific needs are understood, FOX’s highly trained therapists provide a personalized and proactive treatment plan to restore strength, balance, and mobility through exercises that build core strength, increase flexibility, and enhance coordination.

FOX monitors and tests residents’ progress using a process based on Functional Outcome Measures (FOMs) to ensure their therapy plans are producing optimal results. For example, instead of simply measuring a resident’s balance or mobility against their initial assessment, they also measure it against an age normative value – or the average balance and mobility for a person their age. The aim is not only to restore balance and mobility but also to improve it.

“What’s great about collaboration with Coterie is that we have a similar philosophy when it comes to fall prevention,” Blumenthal says. “We don’t want to wait for something to happen and react to it. We want to be proactive. So we always try to identify these very small changes in a resident’s health before they snowball into a much bigger problem that can have serious negative consequences.”


Restoring Life – Alzheimer’s and Dementia Care


FOX shares Coterie’s belief that residents with Alzheimer’s disease and other forms of dementia can enjoy meaningful and rewarding lives – especially when provided with comprehensive therapy services based on research and empathy in a supportive environment. FOX employs a stage-specific approach to dementia, using prescribed interventions to increase the quality of residents’ daily interactions and minimize the challenges they face.

“When it comes to occupational therapy for seniors with Alzheimer’s and dementia, the goal is to focus on what residents can do rather than what they can’t,” Blumenthal says. “And from there, we work to prolong whatever abilities and functionality they have for as long as possible.”

FOX’s occupational therapists evaluate each Hudson Yards memory care neighborhood resident to identify what stage of dementia they’ve reached. This lets them tailor their therapy methods to the resident’s level of cognitive ability with the aims of slowing cognitive decline, maintaining physical ability, and improving quality of life.

For example, FOX and Coterie staff strive to develop a clear idea of what verbal and tactile cues are most effective in communicating with a resident, along with how many instructions a resident can follow in a row, so they can personalize their care plan to minimize incidents that might cause confusion or frustration for the resident.

To reduce fall risks for memory neighborhood residents, FOX takes a hands-on approach in senior physical therapy sessions. Even if residents lack the ability to do strength and balance fitness routines on their own, FOX clinicians can manually exercise their muscles to incrementally improve strength and function.
